A urocele is a swelling of the urinary bladder that balloons outward through the penis. It's usually caused by a blockage or weakness in the tissue that supports the bladder called fascia, which can lead to urine leaking out into the scrotum.

1. Medical Description: A urocele, also known as an inguinal urocele or scrotal urethral diverticulum, is a condition in which there is a herniation of the proximal spongy urethra through a defect in the perineal membrane. The classic sign of a urocele is a visible bulge in the urethra which fills and empties with normal micturition.

2. Causes: A urocele can be congenital or acquired. Congenital uroceles are usually not associated with other urinary tract abnormalities. Acquired uroceles may be secondary to urethral stricture, trauma, overactive bladder or, less commonly, carcinoma of the bladder or prostate.

3. Symptoms: The presentation of a urocele may include a firm, smooth, non-pulsating, and usually painless nodule directly behind and below the perineal raphe. One also might experience difficulty in starting the stream of urine (urgency, incomplete emptying, and spraying) or people who have experienced conditions like spinal cord injuries.

4. Treatment Options: While usually asymptomatic unless the bulging part of the urethra covers the opening of the external urethral orifice, treatment may be needed to address issues such as difficulty in starting to urinate, a slow or weak urinary stream, painful urination, and a post-void residual of more than 100 ml. Treatment options range from increasing fluids to implanting a self-catheterization device, depending on the severity of symptoms.
